Blood work could have been Simmons got pregnant
Simmons and Fitz had a baby... Deke's mother... and Fitz had to hide her. Which is another reason why Simmons needed the implant because she didnt want to remember the Baby. She couldnt do what needed to be done in the past knowing that her baby was out there.
