Several states have banned them. Currently most have an exception for maternal health, but I'm getting feedback that most hospitals and their lawyers are so scared of prosecution that they are waiting until the woman's vitals have dropped low enough to be able to prove she was going to die.

People know they can f up and there are so many safety nets to save them. So many don't try or care. Those who work at something like their life depends on it, usually are successful.
