You will have to choose between "Whole" meaning it never expires as long as you pay
or "Term" which means it's for a certain amount of years (usually max is 10 depending on age)

All depends. 15 years ago I had a 10 year term policy for 550k
I started paying $38 a month and at year 10 they wanted $80 (price isn't fixed on term)
I had to give blood. They also ask you questions about your health and parents health. (say no, no, no....to everything)
5 years ago I got a plan through my job, I had 750k for $50 bucks a month, no blood.
I will assume once you past a threshold of coverage or age they will want blood.
Only the really cheap life insurance policies that pay you 10k-25k (burial costs) don't ask for any type of medical history
Most jobs have a default pseudo type plan where they give you 2.5x your salary but that's not big money to most.
