Coronavirus | 180 deaths following vaccination reported in India

According to a presentation made to the National AEFI Committee during a meeting held on March 31, there have been 617 severe and serious (including deaths) adverse events following immunisation (AEFI). As on March 29, a total of 180 deaths (29.2%) have been reported following vaccination across the country. Complete documentation is available only for 236 (38.3%) cases.

In all, 492 severe and serious AEFI have been classified by the AEFI Secretariat of the Immunisation Technical Support Unit (ITSU) at the Health Ministry. Classification has been completed for 124 deaths, 305 serious events that required hospitalisation, and 63 severe events that did not require hospitalisation.
The classification by the AEFI Secretariat was on the basis of case reporting forms (CRF) and case investigation forms (CIF) submitted at the district level. In the meeting, individual AEFI cases were presented and brought up for discussion by experts in the AEFI causality assessment committee.
Of the 124 deaths, more than 63 deaths (nearly 51%) have been categorised as being caused due to acute coronary syndrome (a range of conditions associated with sudden, reduced blood flow to the heart) or heart attack. Another 11 deaths (12%) of deaths are due to stroke.



As on March 17, the details of the causality assessment of only 13 AEFI including 10 deaths have already been made public by the national AEFI committee. The vaccine was not found to have caused death in any of the 10 vaccinated people.
However, in many cases post mortems have not been conducted. For example, in at least six out of 10 cases where the National AEFI Committee has completed causality assessment, no post mortem has been done, says Malini Aisola, a Public Health Researcher based in Delhi.



Virologist Dr. Jacob John, formerly of CMC Vellore, says that if deaths are not associated with vaccination, then they would be nearly evenly distributed across weeks post vaccination. However, there are 93 deaths in the first three days (31 deaths per day) and 18 deaths in four-seven days (4.5 deaths per day) after vaccination. There have been 11 deaths in 8-28 days (0.5 deaths per day) post-vaccination. “Deaths are not evenly distributed,” he says. There is hence a compulsion to investigate the deaths more thoroughly for any association.
If deaths are seen on a weekly basis, there have been 111 deaths in the first week (nearly 16 deaths per day) but in the next three weeks, there have been only 11 deaths (0.5 deaths per day). “If the deaths are unrelated to vaccination, then deaths should be evenly distributed across weeks after vaccination. There are an extraordinary number of deaths in the first week but in the next two-four weeks there are only about four deaths per week,” says Dr. John.



There have been 59 deaths due to sudden, reduced blood flow to the heart (acute coronary syndrome) or heart attack in the first week but only four in the second-four weeks after vaccination. Similarly, deaths due to stroke are 13 in the first week but only one death in two-four weeks after vaccination. There have been nine “sudden deaths” during the first week but only one death in two-four weeks after vaccination.
In the case of AEFI requiring hospitalisation, there have been 291 AEFI needing hospitalisation in the first week compared with only 13 in two-four weeks after vaccination. “The number of hospitalisations in the first week stands out strikingly different,” Dr. John says. Again, 18 cases of reduced blood flow to the heart (acute coronary syndrome) or heart attack, 10 cases of stroke, and 46 cases of severe allergic reaction (anaphylaxis) in the first week require thorough investigation.
Weakness in one, two or all four limbs (mono/para/quadriparesis) seen in 17 people hospitalised is similar to transverse myelitis seen during the trial in the U.K., says Dr. John. About 15 cases of seizure in the first week and no such cases in the two-four weeks after vaccination is abnormal, he says. “There is something going on in the central nervous system. Also, 17 cases of mono/para/quadriparesis in four weeks needs thorough investigation,” he says.
According to him, 59 cases of severe AEFI not requiring hospitalisation in the first week and four such cases in two-four weeks may be a signal.
“Since Covishield is the same vaccine as AstraZeneca, updated warnings related to these rare conditions, information for vaccine recipients and the public about when to seek medical attention, and information for health providers about how to identify and treat such occurrences needs to be done for the vaccine,” says Ms. Aisola.
The European Medicines Agency (EMA) has included only six deaths from India after vaccination with Covishield for its analysis. “Due to a massive backlog in processing assessments in India, we have reported the data internationally for just a tiny fraction of the actual cases that have occurred,” she says. “Timely assessment is important to formulate recommendations for vaccination for particular groups and guidance for any groups that may be identified as more susceptible to serious AEFIs. It is also important for revising and updating screening procedures and to ensure that arrangements for treatment are put in place.”
“The National Committee is depending on evidence and investigations conducted at the local level. However, we observed a strong tendency by local authorities to immediately rule out any links to vaccination, even before investigation has been conducted,” says Ms. Aisola. “There is an urgent need to strengthen AEFI investigation at the local level where protocols may not be adhered to and the quality of evidence being collected is often weak and inadequate.”

With about a third of people in the U.S. with one dose and about 20 percent fully vaccinated against the novel coronavirus, people are wondering why they still need to be cautious in their daily activities. This has to do with how the vaccine works and being careful during this stage as experts are gathering more data on how immunization is affecting case loads.

The goal and purpose for vaccination is to prevent severe illness and hospitalization. The vaccines that currently have emergency use approval, Pfizer/BioNTech, Moderna, and Johnson & Johnson, have not been shown to prevent infection.

This means that people who are vaccinated may be able to get infected and could transmit the virus to others, although the probability of that happening might be lower than that of unvaccinated people. Experts are focused on gathering more data to see if transmission is happening. There’s also limited data on how the vaccine works for people who are immunocompromised and how well it protects against the coronavirus variants.

Since vaccinated people may still be able to shed virus, wearing masks in public spaces will be important for keeping others safe from exposure. Keeping distance between others will also keep others safe. All these health guidelines are meant to reduce transmission.


There have been changes in recommendations for what people who are fully vaccinated can do. The guidelines from the Centers for Disease Control and Prevention (CDC) were updated to say that people who were fully vaccinated could spend time unmasked indoors with other fully vaccinated people. “Though the risk of disease may be minimal to the fully vaccinated person themselves, they should be mindful of their potential risk of transmitting the virus to others if they become infected, especially if they are visiting with unvaccinated people at increased risk for severe illness from COVID-19 or who have unvaccinated people at increased risk for severe disease in their own households,” according to the CDC guidelines. The risk of transmission in these settings is low enough for that and since all parties are vaccinated the risk of developing severe illness is also low.

It’s also important to note that vaccination progress varies from state to state, with some estimated to have vaccinated fewer than 45,000 people out of every 100,000 and others achieving 50,000 to 60,000, according to the CDC vaccine tracker. These vaccination rates affect your risk when you go out to do grocery shopping, take public transport or spend any time indoors with people outside your household or bubble. It affects the chances that you could be standing next to someone who is unvaccinated and/or infected. It’s also something to keep in mind if you are traveling domestically.

Because the coronavirus is still circulating in the population, including variants, it’s important to try to reduce transmission rates as much as possible. Cases are also spiking in several states, meaning that even as people get vaccinated, expanding interactions with more people could expose more people to the virus and keep transmission going.

Meanwhile, the Centers for Disease Control and Prevention said the variant of coronavirus that originated in the United Kingdom, the B.1.1.7 variant, has become the dominant strain in the U.S. Experts believe the variant is more contagious and could be more deadly, but all three vaccines authorized in the U.S., including Moderna’s, have shown to work effectively against the strain.

Pharmaceutical giant Pfizer has kicked off early stage clinical trials of an experimental oral antiviral drug to treat COVID-19.

The treatment is a potent protease inhibitor, the same kind of technology used to treat HIV and hepatitis C. Protease inhibitors bind to a viral enzyme and prevent the virus from replicating in the cell.

Pfizer said preclinical studies showed the oral antiviral “demonstrated potent in vitro anti-viral activity against SARS-CoV-2, as well as activity against other coronaviruses.” That means the treatment could be useful against future coronavirus threats.

“Tackling the COVID-19 pandemic requires both prevention via vaccine and targeted treatment for those who contract the virus. Given the way that SARS-CoV-2 is mutating and the continued global impact of COVID-19, it appears likely that it will be critical to have access to therapeutic options both now and beyond the pandemic,” Mikeal Dolsten, Pfizer chief scientific officer, said in a statement.

Dolsten said the oral therapy could be prescribed to a patient at the first sign of infection, without requiring they be hospitalized or in critical care.
Pfizer is also studying an intravenous antiviral therapy to treat COVID-19 that is being used in a clinical trial involving hospitalized patients.
“Together, the two have the potential to create an end to end treatment paradigm that complements vaccination,” Dolsten said.

Pfizer, in collaboration with German company BioNTech, was the first pharmaceutical company to receive emergency use authorization from the Food and Drug Administration (FDA) for a COVID-19 vaccine.

Currently, the only FDA-approved antiviral treatment for COVID-19 is Gilead’s Remdesivir.

Pfizer says it will share pre-clinical data on its experimental pill on April 6.

Karkala, Apr 8: In a tragic incident, a four-month-old child died hours after it was administered a regular vaccine. The father of the child Sudhakar R Shetty has filed complaint in this regard at Karkala town

The child was administered regular vaccination at Renjala health care center on Wednesday April 7. The parents had taken the child back home after vaccination.
After a few hours of taking the dose, the child started vomitting and fell ill. Immediately, it was taken to Rotary Hospital in Karkala where the doctors declared the child dead.
An unnatural death case is filed in this regard at Karkala town police station.

The family of a young Israeli woman who died of heart inflammation after being vaccinated against COVID-19 has blamed the hospital where she was treated for her death.

Shirel Hilel, 22, received her second vaccine dose two weeks ago, her sister Nofar told Channel 13 news, in a segment aired Thursday evening. She later reported feeling unwell and said she was experiencing muscle pain.


“I told her, ‘take [a pain killer]. No help? Tomorrow go to the doctor,'” Nofar said.

However, Hilel’s condition worsened and she began to have difficulty breathing and pains in her chest and was taken to Safed’s Ziv Medical Center.

At the hospital, Hilel was diagnosed with myocarditis, inflammation of the heart muscle, which boyfriend Eyal said the doctor compared to a swollen shoulder.

“I said okay, three, four days or a week. I’ll be with her as much as needed,” he said.

According to the network, there have been other cases of myocarditis among Israelis who received both vaccine shots, but all had been successfully treated, including patients in more serious condition than Hilel.

Hilel was eventually taken to the cardiac care unit, where her family asserted she was not sufficiently watched over by hospital staff.

“I looked at the monitor, I saw the breaths. The monitor was beeping and no one approached,” Nofar Hilel said.

Two days later, she said, her sister collapsed.

“I got ready so quickly and said, ‘I’ll go up [to the hospital] to see what we can save.’ Five minutes later they called to say she died,” Nofar said.

She added: “She just deteriorated before their eyes and no one, just no one even lifted a hand and said, she isn’t receiving care.”

A lawyer for the family said Hilel’s relatives intend to take legal action against the hospital.

Ziv Medical Center, meanwhile, said Hilel received similar treatment to other patients who had recovered within days.

“On the last day of hospitalization, a deterioration in her situation began. Persistent heart rhythm disturbances developed that didn’t respond to care and after continued resuscitation, she was pronounced dead. We share in the sorrow of the family over the heavy loss,” it said in a response to the network.

Last month, the Health Ministry instructed hospitals to report any cases of heart inflammation among the vaccinated.

“After several such cases of this type happened close to receiving the vaccine, it was decided to put a specific focus on this issue,” Tal Brosh, head of the Infectious Disease Unit at Assuta Medical Center in Ashdod, told the Ynet news site at the time.

He stressed it remained unclear if there was any direct connection between the cases of heart inflammation and the vaccine.

According to the latest Health Ministry figures, 5,252,718 Israelis have received at least one vaccine dose and 4,804,891 have gotten both shots.

The vaccine will drop the death count pretty quickly. The concern is what you been saying for a while. Will a variant be created that the vaccine can’t protect from.

The high number of cases is eye opening. I think people will relax as the death count drops and allow the cases to remain high and we get surprised by a variant which starts the cycle over again
