I don't see the issue as being a "self esteem" issue - for example, I think pretty highly of myself, both inside and outside the bedroom. However, I have been with partners where I don't feel nearly as comfortable to say, "Hey, what you doing down there?" as others. I've also been in a position where a man continued to try and force himself on me even after I expressed to him things weren't pleasurable for me.
