I agree scarring probably could be edited out, Plus with sites like FTV, Hegre and others that focus on softcore erotic art, I'm sure the edit out most blemishes.
My assumption was based on Kina Kara whom like her has small nipples, but fairly large breasts. For along time nobody could guess as to whether her breasts where real or fake until I found video where the camera pans to show an under shot which revealed the scars.

Well, actually implants can also go in through incisions through the axilla (armpit) and through an incision in the navel (believe it or not).
