I dont know about all the situations, but I remember the porn star Xtacee did a scene with dude (Bill).
He was supposed to give her a facial and she got mad, which ended up in him smacking the shit out of her.
The part where he smacks her was edited it out in many of the vid clips,
but there was an unedited version here on the board a long time ago.
On a sidenote, Xtacee was known for being really hard to work with.
As a result, she was blackballed by many directors in porn (no pun intended).
