Online Application Creation Help

Found a existing PDF for a job application. Exactly, verbatum, what I need. Wanted to copy and paste and call it a day, but I don't know what software or program I would need. I tried to convert it to a word doc to customize it, but when I switched it back to PDF format it was all funky.
Aspired to create a PDF that people could fill out online, anyhelp?

Thank you this helped a little bit, but Im looking for something where I can take the existing text off of a PDF form and replace it with the same font, same size, and same style. For example, if the og PDF reads Simon Front Properties, I want to be able to go in there and replace it with Mafioso81 Properties, without it losing any integrity.
