B. if the ingredients in the soap are all heated to the same temp or melting point to pour into the mold means they will all cool at the same temp so they should separate that much.
C. a full bar or soap lathers differently in warm/cold and hard/soft waters.

The ingredients of soap are added to the mold at the same temperature but that does not necessarily mean that they'd all cool at the same rate. This is the basis of fractional crystallization and selective precipitation from solutions.
I don't think it's because of some inhomogeneity in the soap that causes it to lather differently at different stages.
Remember, in addition to the surfactant which the soap is, there are other surfactants and additives for specific functions one of which is foam control.
I'd be inclined to believe that it's because of a surface film of "scud" or some particle contaminant (dirt) that could destabilize the foam.
If you use an abrasive sponge with your soap perhaps you'd solve the problem?
