Raye's removal hasn't fixed 49ers' communication problems

One of the biggest reasons for the pre-Week Four firing of 49ers offensive coordinator Jimmy Raye was reportedly Raye's inability to clearly communicate play calls. Raye's problems allegedly ranged from garbling verbiage to simply taking too long to relay plays to quarterback Alex Smith.
The 49ers appear to have new communication issues, and they're more visible on the field.
According to Tim Kawakami of the San Jose Mercury News, backup quarterback David Carr was so much under the impression he'd replace struggling starter Alex Smith in the fourth quarter of Sunday night's game that Carr walked out to the huddle at roughly the 11-minute mark. Smith went onto lead a touchdown drive capped by tight end Vernon Davis' seven-yard scoring reception to cut Philadelphia's lead to 24-17.
But, apparently, no member of San Francisco's coaching staff informed Carr he wasn't going in.
Prior to the on-field miscommunication, Smith was showered in boos by the home crowd.
The Eagles, meanwhile, are dealing with multiple injuries to key players. Wide receiver DeSean Jackson exited the game temporarily with a knee injury. Left tackle Jason Peters is out with his own knee injury. Defensive end Trent Cole has missed snaps with a hamstring injury.
The 49ers are currently in a goal-to-go situation with just over two minutes left. Perhaps Smith will give them a chance to get their first win.
But we wouldn't bet on it.
UPDATE: The 49ers scored on a one-yard touchdown pass to running back Frank Gore. The score is now 27-24 Eagles.
We're going out on a limb and still betting on an 0-5 start for Mike Singletary's team.
UPDATE II: Smith threw a game-ending interception. 0-5 it is.
