CDC director warns of 'impending doom' as Covid-19 cases spike in most states

(CNN)Much of America's recent progress against Covid-19 has been erased as new infections jump nationwide.

Now the director of the Centers for Disease Control and Prevention said she's afraid of what will happen next.

"What we've seen over the last week or so is a steady rise of cases," said CDC Director Dr. Rochelle Walensky on Monday.

"I know that travel is up, and I just worry that we will see the surges that we saw over the summer and over the winter again."

But now, the troubling B.1.1.7 variant strain is spreading more rapidly in the US. That strain isn't just more contagious, health experts say. It appears to be deadlier as well.

And the combination of young, carefree revelers and states ditching safety mandates has helped send the country backward, said Dr. Anthony Fauci, director of the National Institute of Allergy and Infectious Diseases.

"What we're likely seeing is because of things like spring break and pulling back on the mitigation methods that you've seen now," Fauci told CBS' "Face the Nation" on Sunday.

The great news is all three vaccines being distributed in the US appear to work well against the B.1.1.7 strain. But with only 15.8% of the US population fully vaccinated -- and anti-vaxxers and vaccine hesitancy preventing America from returning to normal faster -- it's time for a reality check.

"Now is one of those times when I have to share the truth, and I have to hope and trust you will listen," Walensky said.

"I'm going to reflect on the recurring feeling I have of impending doom ... We have so much to look forward to, so much promise and potential of where we are and so much reason for hope. But right now, I'm scared."

Before she became CDC director, Walensky was on the front lines of the pandemic, witnessing some patients die from Covid-19.
"I know what it's like as a physician to stand in that patient room -- gowned, gloved, masked, shielded -- and to be the last person to touch someone else's loved one, because they are not able to be there," she said.

The US has come "such a long way," Walensky said, pleading with all Americans to keep masking up and "hold on a little while longer" as more people get vaccinated.

Young people are fueling much of this new surge

At least 27 states have averaged at least 10% more cases each day this past week compared to the previous week, according to Johns Hopkins University.

"A lot of the spread is happening among younger people," said Dr. Ashish Jha, dean of the Brown University School of Public Health.

"That's the group that is moving around, kind of relaxing, getting infected."

Spring break crowds swelled in popular beach destinations, including South Florida, where local leaders said the vacationers were more than they could handle. Air travel has broken pandemic records this month, with millions of Americans boarding planes.

And some state governors and local officials recently relaxed safety mandates, despite warnings from health experts to keep them in place a bit longer.

"We're weeks away from a point where we can begin to do these things a bit more safely," Jha said. "But I think states have just moved too fast."

Michigan is already suffering another Covid-19 surge, driven largely by young people ages 10 to 19, said Dr. Joneigh Khaldun, the state's chief medical executive.

Khaldun attributed the rise in cases to a number of factors, like more gatherings, more mobility, business reopenings and specific outbreaks in some prisons and schools.

Illinois officials are planning to deploy rapid response vaccination teams to several counties and expand vaccine eligibility after a "concerning possible trend in increasing COVID hospitalizations and case rates."

"Recent increases in hospital admissions and test positivity are concerning new developments and we don't want to go down the same path we've seen before and experience a resurgence in the pandemic," Dr. Ngozi Ezike, director of the Illinois Department of Public Health, said in a statement.

"We cannot move forward if our metrics are going backward."

Vermont just reported its worst day for new cases reported since this pandemic started, with more than 250 new infections reported Friday.

The increase is likely fueled by more people moving around in warmer weather and by variant strains of coronavirus, said Dr. Mark Levine, Vermont's state health commissioner.

"Our efforts to vaccinate Vermonters is a race against what the virus does best: move easily from person to person," Levine said.

"Throughout the country, including up and down the Eastern Seaboard, case numbers are up."

New Hampshire has recently suffered not just an increase in new cases, but also increasing test positivity rates. And the number of infections is increasing among young people, especially in teenagers and young adults, officials said.

Gov. Chris Sununu said he expects New Hampshire to endure a "spring surge."

New data shows vaccines are highly effective

But there's great news on the vaccine front: The CDC says the Pfizer/BioNTech and Moderna vaccines offer strong protection -- not just against illness, but also against infection.

Both vaccines require two doses. After full vaccination, both vaccines were 90% effective at preventing infections, including infections that had no symptoms, according to a study in the CDC's Morbidity and Mortality Weekly Report.

With just one dose, the vaccines were about 80% protective, the report said.

The study looked at how the vaccines protected nearly 4,000 health care workers and first responders. The volunteers in the study worked in eight locations around the country and had been observed from mid-December to mid-March.

The CDC tested volunteers routinely, regardless of symptoms.

"Reducing the risk for transmissible infection, which can occur among persons with asymptomatic infection or among persons several days before symptoms onset, is especially important among health care personnel, first responders, and other essential and frontline workers given their potential to transmit the virus through frequent close contact with patients and the public," the report said.

North Carolina Professor Die's Shorthly After Johnson & Johnson Shot,Woman raises questions after mom's death certificate references vaccine. Experts warn about drawing conclusions.

A Chapel Hill woman who says her mother died of a stroke 48 hours after receiving the Johnson & Johnson COVID-19 vaccine spoke to WRAL News about her concerns and the questions she has about her mother's death. The state and a UNC doctor, though, warn about drawing any conclusions yet.

Becca Ingle is understandably devastated after her mother's sudden death two weeks ago. She says her mother was in good health, but started feeling very ill shortly after getting the vaccine. She wants to know if her mother's death could be connected to the vaccine. The state is investigating this case and so is WRAL.

As a college professor who worked in person with her students at Appalachian State University and spent lots of time caring for her two grandchildren, 63-year-old Virginia Ellington was counting the days until she could be vaccinated against COVID-19.

"I have texts from her saying how excited she was to get it," her daughter, Ingle, said.

Ingle said her mother and father got the Johnson & Johnson vaccine on the morning of Monday, March 8, at the Watauga Health Center in Boone.

Ellington started feeling very tired and even asked her husband about possible vaccine side effects, Ingle told WRAL News.

Ellington went to work the next day but was still feeling unwell, according to her family. On Wednesday, March 10, she stayed in bed. A little while later, her husband returned to the bedroom to find her unresponsive.

"He tried giving her CPR, and he called 911, and they said she was gone when they got there," Ingle said.

Ingle said the local doctor, who did a preliminary examination, told the family that her mother died of a stroke. The local medical examiner, a paramedic and a teacher who signed the death certificate, wrote that the vaccine was one of the "significant conditions contributing to her death."

"We were all shocked when we saw that was even on there," Ingle said.

Ingle shared her questions about the vaccine and her mother's death on TikTok. The post has had more than 4 million views, but it received mixed reviews. TikTok even placed a warning on it.

"It just breaks my heart that this is what happened and this is what it takes to share her story," Ingle said.

Ingle says her mother was in good health prior to her death.

"It (the vaccine) definitely escalated things," she said. "Whether she was going to have a stroke in 30 years or whether she was going to have it now, it definitely triggered it."

Ingle says high blood pressure and strokes run in her family.

Dr. Stephan Moll, a medical professor, researcher and hematologist at UNC Health, believes connecting the vaccine to a stroke is a rush to judgment.

"The vaccine is appropriate for everybody independent of the family history or their personal history," he said.

He thinks mention of the vaccine should not have been included on the death certificate.

According to the Centers for Disease Control and Prevention, 795,000 people in the United States have strokes every year. North Carolina has one of the highest rates, at 26,000 per year, or 70 per day.

"Some people will develop a stroke and have had the vaccine," Moll said, "but that's a coincidental occurrence. Even an association, that's a rare one. It should not sway away anybody from getting the vaccine."

The CDC has found no evidence of a connection between the vaccines and strokes. Moll says the only way to know for sure is to do an autopsy.

The North Carolina Department of Health and Human Services has referred Ellington's case to the Office of the State Medical Examiner for an autopsy. Agency officials told WRAL News:

"The death investigation for this case is in the preliminary stage, which is why the death certificate says 'pending.' After an autopsy examination by one of our regional autopsy centers, the preliminary results do not show that this death was related to a vaccine or vaccination."

Susan Hawkins, the local medical examiner who signed the death certificate and made the reference to the vaccine as a factor, declined comment to WRAL News and referred all questions to the state.

The final state medical examiner's report could take several months to complete, and more answers should be clear at that time.

That portends a painful wait for a daughter seeking answers.

"I’m not here to say you shouldn’t take it," Ingle said. "I’m just saying raise awareness. You should check on your loved ones if you are getting it."

Johnson & Johnson provided this statement to WRAL News:

"There is no greater priority than the safety and well-being of the people we serve. We carefully review reports of this nature that involve individuals receiving our medicines, or in this instance, a vaccine."

Company officials go on to say that these reports are evaluated by the U.S. Food and Drug Administration and other health authorities that are monitoring the overall safety of the vaccine rollout.

Recognizing this story may generate a lot of interest, the family issued a statement saying, “As the family mourns their loss, please respect their need for privacy.”

Tributes to 'amazing' dad and Nottingham Forest fan who died days after getting coronavirus vaccine

Tributes have been paid to an "amazing" man who died of coronavirus just days after receiving his vaccine.

Stephen Cunningham, 66, from Clifton, was classed as extremely clinically vulnerable as he had heart problems and type 2 diabetes.

He died on February 21 this year in Nottingham City Hospital after a short battle with Covid-19. He leaves behind his wife Glenis, who he was married to for 35 years, and their two children.

The family have raised concerns about the quality of care Mr Cunningham received while he was being treated in hospital.

Melissa Cunningham, Stephen's daughter, has described him as a family man with a passion for sports - especially his beloved Nottingham Forest.

"He was amazing," she said.

"Just an amazing man. He loved his football - absolutely loved Nottingham Forest.

"He loved motor racing as well, and Lewis Hamilton. He was just a family man - a loving and caring man.

"As a dad, he was very protective - always very protective. He used to work away a lot when I was growing up.

"He was shielding and didn't go out for a year before he received his Astra Zeneca vaccine."

However, just five days after receiving his first dose, Stephen was admitted to hospital where he then tested positive for Covid-19.
The family was rang by the hospital at 9am on February 21 telling them to they needed to get there and see him. However, just 10 minutes later, they received another call to say Stephen had passed away.

According to the government website, it takes a "few weeks" for the body to build up immunity to the virus after receiving the vaccine.

The family have told Nottinghamshire Live they plan to make a formal complaint with the NHS trust.

They have criticised the standard of care he received, saying he was not transferred to the ICU unit before his death. They say he frequently called home to tell them he was struggling with his breathing and was not being seen.

Melissa also says he did not receive an X-Ray for a possible broken hip while in hospital after suffering a fall.

A person who tested positive after taking the Covid-19 vaccine has died at the Jorhat Medical College and Hospital (JMCH) in Upper Assam’s Jorhat district.

Jorhat deputy commissioner Roshni A Korati confirmed that Sankar Prasad Khound, who had taken the vaccine, died of Covid-19 on Tuesday.


Khound is a resident of Atila Gaon in the district.

A total of four people who had received the Covid-19 vaccine at JMCH later tested positive for the virus on March 13 and 14.


According to Khound’s son Arnab, his father had been suffering from dysentery and undergone a Covid-19 test on March 13, where he along with his mother tested positive



Arnab’s parents had received the vaccine shot on the same day and after testing positive were admitted to the JMCH.

The other couple Partha Saikia and his wife, who had been vaccinated with the Khounds had also tested positive and they were admitted to the hospital.

Meanwhile, three more persons have tested positive for Covid-19 after being vaccinated here.

They include CISF personnel Dimbeswar Saikia (51) posted at Jorhat ( Rowriah) Airport, Pranjal Saikia the personal security officer to the general observer Manjit Singh Brar, who had also tested positive and had to be replaced.

An intern of the hospital, who had been vaccinated also tested positive for the virus and was admitted to the hospital.

Abstract
Real-world evidence (RWE) and randomized control trial (RCT) data are considered mutually complementary. However, compared with RCT, the outcomes of RWE continue to be assigned lower credibility. It must be emphasized that RWE research is a real-world practice that does not need to be executed as RCT research for it to be reliable. The advantages and disadvantages of RWE must be discerned clearly, and then the proper protocol can be planned from the beginning of the research to secure as many samples as possible. Attention must be paid to privacy protection. Moreover, bias can be reduced meaningfully by reducing the number of dropouts through detailed and meticulous data quality management. RCT research, characterized as having the highest reliability, and RWE research, which reflects the actual clinical aspects, can have a mutually supplementary relationship. Indeed, once this is proven, the two could comprise the most powerful evidence-based research method in medicine.

INTRODUCTION
Real-world data (RWD) refer to data collected from diversified areas of daily life that are outside the scope of highly controlled randomized control trials (RCTs).1 Examples of RWD are the large quantities of data collected broadly through the use of a diverse range of medical devices by various users, including self-checked glucometer or sphygmomanometer. These real, practical data cannot be obtained through contemporary or traditional clinical trials. A wide range of data already accumulated, including the electronic medical records (EMRs) used in hospitals,2,3,4 Korea National Health and Nutrition Examination Survey (KNHANES) data,5 Health Insurance Review and Assessment service data6 or Korea Adverse Event Reporting System (KAERS) database7 are also included in RWD.
EMR, in allowing for the accumulation of a large amount of clinical data, are increasingly used for medical research.8,9 Worldwide, EMR-based clinical research has been reported to increase the quality of research and satisfaction of clinicians.10,11,12,13 Depending on study design and objectives, EMR research may utilize a variety of data: medication prescription, treatment decision making, disease management, and clinical research data. For these reasons, EMR data are recognized as definitive data with the highest reliability among RWD. Analysis of RWD yields real-world evidence (RWE), and many scholars are focusing their interest on RWE research using RWD accumulated broadly, along with the issue of big data. Efforts have been made toward expanding the range of relevant clinical research.
Fundamentally, it is desirable for the new research format, RWE, and the traditional research format, RCT, to be mutually complementary. However, despite the comprehensive reports on the importance of RWE, there is a tendency among scholars of mistrusting the findings of RWE compared with RCT. For many researchers, overcoming a diverse range of biases that can occur in EMR-based RWE research requires trial and error. Therefore, we intend to present opinions on precautions that can be used effectively in executing research using RWE from the clinical rather than technical perspectives.

RWE VERSUS RCTs
RCT conducts research on highly selective populations and is managed in tightly controlled settings.14 RCTs are conducted under the premise that the research outcomes of the chosen sample population represent those of the entire population. This research format is deemed to have the highest reliability. The majority of clinical guidelines are given on the basis of the resultant values deduced from RCTs.15 However, a disadvantage is that participants may fail to reflect the actual clinical site sufficiently to represent the entire population, given the clear set of inclusion or exclusion criteria. This drawback of exclusion has been deemed a bias.16,17 That is, there would be difficulty in applying the resultant values of data analysis confirmed in a small number of patients selected in accordance with set criteria in RCTs to diversified actual situations that could occur in the real world. For this reason, the foundation on which to elevate definitive reliability through concurrent execution of RWE research, in addition to RCT, needs to be established; these two should ideally be developed in a mutually supplementary rather than competing relationship. For example, if the RCT concentrates on the efficacy of a drug, the RWE can focus on the epidemiology, effectiveness, safety, or costs of treatment related to the drug (Tables 1 and and22).18 RWE research that can sustain follow-up observation after the completed RCT research holds important significance. Clinical trials and secondary syntheses of evidence-based information can be biased owing to undisclosed financial and nonfinancial conflicts of interests. Therefore, research should be undertaken with the perception that managing these conflicts of interests more thoroughly is crucial to maintaining the essence and credibility of research.19

Placer County sheriff linked vaccine to death despite experts' cautions

SACRAMENTO, Calif. - A California sheriff announced in January that a man died hours after receiving a COVID-19 vaccine even though his county’s health officials said the declaration was premature and detrimental.

The announcement by Placer County Sheriff Devon Bell on Facebook quickly spread around the world and was used by anti-vaccine activists to try to discredit COVID-19 vaccinations.




The man, identified only as a 64-year-old health care worker, died Jan. 21 after receiving the shot in Auburn, northeast of Sacramento in the foothills of the Sierra Nevada.

Eventually, it would be determined that the vaccination was coincidental to the death, but at the time the sheriff’s statement on Facebook said the man had received a shot "several hours before their death."


A review of county emails show an intense internal debate about the death, the language of an announcement, how an announcement could feed fears about the vaccine and the intent of the Sheriff’s Office to proceed with an announcement.

"We strongly believe it is premature to do any public messaging surrounding this death," Dr. Rob Oldham, director of Placer’s Health and Human Services agency, said in one email to Undersheriff Wayne Woo. "We can see very little benefit to public health in doing so and substantial potential detriment."

Oldham, however, agreed in the Jan. 23 email to help draft an announcement that made clear the official cause of death was still pending.

The announcement became widely reported news nonetheless.

There was also backlash on the Facebook page from people who commented that releasing information prematurely was harmful to the goal of mass vaccinations. The office’s replies included telling one commenter to "feel free to unfollow this page."

The sheriff apologized a week later in a statement that said the man’s death was unrelated to the vaccine.

"We have learned that not only had he recently been diagnosed with COVID-19, he also had underlying health issues, and had been exhibiting symptoms of illness at the time the vaccine was administered," the statement said. "Clinical examination and lab results have determined the COVID-19 vaccine has been ruled out as a contributing factor in the individual’s death."

An infectious disease expert told the Bee it was irresponsible for the sheriff to make the announcement.

"That could potentially lead to a number of people choosing not to get vaccinated, who then get infected, who then potentially even die," said Dr. Jake Scott of Stanford University. "So this is a matter of life and death."

Another medical expert said that when doing things to a large number of people — "whether it’s vaccinate them or tap them on the shoulder" — some will die soon.

"Simply observing that somebody was vaccinated and then had something bad happen, hours or days later, doesn’t mean it was the vaccine that caused it," said Dr. Arthur Reingold, head of the epidemiology division at the University of California, Berkeley.

The Sheriff’s Office defended its position.

"As a result of the compelling information, the Placer County Sheriff’s Office felt it was important to inform the community of this significant incident," spokeswoman Angela Musallam said in an email.

The Bee said Oldham declined an interview request but emailed a statement.

"We value our partnership with the Placer County Sheriff’s Office Coroner Division, and look forward to continued collaboration and working toward shared solutions with them," it said.

Pregnant Mother and baby Die's Right After Taking Covid19 Vaccine
Noa Lifshitz-Shaked, mother of three, dies due to hemorrhage, together with 36-week fetus. 'A talented, bright, woman with a love of life.'
Noa Lifshitz-Shaked, 37, died at 36 weeks pregnant due to internal hemorrhage.


Lifshitz-Shaked, a resident of Hod Hasharon in central Israel and a mother to 3-year-old twins, was brought for urgent medical treatment to Meir Medical Center in Kfar Saba, partially conscious and suffering from very severe internal hemorrhage which endangered her life.

Her condition was initially noticed by her father, who is by profession an operating room nurse.

Upon her arrival in the hospital, she underwent an emergency Caesarean section to deliver her baby, who was born in serious condition. While she was still in the operating room, hospital staff succeeded in stopping the hemorrhage and resuscitating the mother.

Afterwards, staff arrived from Tel Aviv's Ichilov Hospital, hooked her up to an ECMO machine, and transferred her in critical condition to their general intensive care unit.

However, despite the Ichilov staff's best efforts, which included two complex surgeries to save her life, Noa died a few hours ago, on Thursday morning. The baby, who was still in NICU at Meir, died during the course of Wednesday night.
Lifshitz-Shaked's family said: "Noa was an amazing mother to Lani and Lori, and married with great love to Ido. She was a beloved sister to Lilach and the best daughter in the world to her parents, Tzippi and Ze'evik. Noa aimed for excellence in everything that she touched, with outstanding honesty and integrity. She was a talented and bright woman, with a great love of life."
"At this difficult time for our family, we would like to thank the medical staff at Ichilov for fighting uncompromisingly for her life and the life of the baby. We will remember and love her forever, and we will continue her legacy and raise her daughters with great love."

SACRAMENTO, Calif. - A California sheriff announced in January that a man died hours after receiving a COVID-19 vaccine even though his county’s health officials said the declaration was premature and detrimental.

The announcement by Placer County Sheriff Devon Bell on Facebook quickly spread around the world and was used by anti-vaccine activists to try to discredit COVID-19 vaccinations.




The man, identified only as a 64-year-old health care worker, died Jan. 21 after receiving the shot in Auburn, northeast of Sacramento in the foothills of the Sierra Nevada.

Eventually, it would be determined that the vaccination was coincidental to the death, but at the time the sheriff’s statement on Facebook said the man had received a shot "several hours before their death."


A review of county emails show an intense internal debate about the death, the language of an announcement, how an announcement could feed fears about the vaccine and the intent of the Sheriff’s Office to proceed with an announcement.

"We strongly believe it is premature to do any public messaging surrounding this death," Dr. Rob Oldham, director of Placer’s Health and Human Services agency, said in one email to Undersheriff Wayne Woo. "We can see very little benefit to public health in doing so and substantial potential detriment."

Oldham, however, agreed in the Jan. 23 email to help draft an announcement that made clear the official cause of death was still pending.

The announcement became widely reported news nonetheless.

There was also backlash on the Facebook page from people who commented that releasing information prematurely was harmful to the goal of mass vaccinations. The office’s replies included telling one commenter to "feel free to unfollow this page."

The sheriff apologized a week later in a statement that said the man’s death was unrelated to the vaccine.

"We have learned that not only had he recently been diagnosed with COVID-19, he also had underlying health issues, and had been exhibiting symptoms of illness at the time the vaccine was administered," the statement said. "Clinical examination and lab results have determined the COVID-19 vaccine has been ruled out as a contributing factor in the individual’s death."

An infectious disease expert told the Bee it was irresponsible for the sheriff to make the announcement.

"That could potentially lead to a number of people choosing not to get vaccinated, who then get infected, who then potentially even die," said Dr. Jake Scott of Stanford University. "So this is a matter of life and death."

Another medical expert said that when doing things to a large number of people — "whether it’s vaccinate them or tap them on the shoulder" — some will die soon.

"Simply observing that somebody was vaccinated and then had something bad happen, hours or days later, doesn’t mean it was the vaccine that caused it," said Dr. Arthur Reingold, head of the epidemiology division at the University of California, Berkeley.

The Sheriff’s Office defended its position.

"As a result of the compelling information, the Placer County Sheriff’s Office felt it was important to inform the community of this significant incident," spokeswoman Angela Musallam said in an email.

The Bee said Oldham declined an interview request but emailed a statement.

"We value our partnership with the Placer County Sheriff’s Office Coroner Division, and look forward to continued collaboration and working toward shared solutions with them," it said.

Noa Lifshitz-Shaked, 37, died at 36 weeks pregnant due to internal hemorrhage.


Lifshitz-Shaked, a resident of Hod Hasharon in central Israel and a mother to 3-year-old twins, was brought for urgent medical treatment to Meir Medical Center in Kfar Saba, partially conscious and suffering from very severe internal hemorrhage which endangered her life.

Her condition was initially noticed by her father, who is by profession an operating room nurse.

Upon her arrival in the hospital, she underwent an emergency Caesarean section to deliver her baby, who was born in serious condition. While she was still in the operating room, hospital staff succeeded in stopping the hemorrhage and resuscitating the mother.

Afterwards, staff arrived from Tel Aviv's Ichilov Hospital, hooked her up to an ECMO machine, and transferred her in critical condition to their general intensive care unit.

However, despite the Ichilov staff's best efforts, which included two complex surgeries to save her life, Noa died a few hours ago, on Thursday morning. The baby, who was still in NICU at Meir, died during the course of Wednesday night.
Lifshitz-Shaked's family said: "Noa was an amazing mother to Lani and Lori, and married with great love to Ido. She was a beloved sister to Lilach and the best daughter in the world to her parents, Tzippi and Ze'evik. Noa aimed for excellence in everything that she touched, with outstanding honesty and integrity. She was a talented and bright woman, with a great love of life."
"At this difficult time for our family, we would like to thank the medical staff at Ichilov for fighting uncompromisingly for her life and the life of the baby. We will remember and love her forever, and we will continue her legacy and raise her daughters with great love."
